That was part of the plotline of "Conquest of Space", which I watched over the weekend. The commander loses his grip on reality and tries to scuttle the mission, on religious grounds. A 1955 plotline. She probably wasn't a danger to any mission she was involved with, unless her love rival was part of the crew, and I don't think she was in the astronaut corps. I don't think there is any way to screen a person so effectively as to rule out all possibility of future instability. I'm sure NASA does the best psych screening money can buy. The sad part is, the astronaut she was in love with will probably have to resign as well.
