Chuck, yup, the same thing happened to me. I called the 800 number 1 1/2 months ago to have a copy sent, which they said they would do, but nothing ever happened. I called again a couple weeks ago; their response was that they were out of stock in the past, but were in stock now and they would send a copy - nothing received yet. Finally gave up and made a B/W copy at the library.
