The image is generated by a perl script by Stephen Amadei called tcview.pl, I got it with a group of other scripts, so I'm not positive what files are required for it and what aren't. Configuration of the script was fairly easy, you have to fill in community strings for snmp and give it some basic information about how your particular rack is setup. I got the link from a post on the list, so searching the archives should yield something. I wasn't able to find a download listed on google.
