I'm not convinced by what you point out that it is faked. The file is a 338x450pixel jpg., probably highly compressed. Of course if you zoom into it you are going to see all sort of artifacts and distortions. These are particularly noticeable at boundaries and tonal transitions. They are present throughout the image, not just where the "tattoo" is.

That said, I am not convinced that it is either real or faked.

I believe this has been faked.  The fractal seems to have been added to a normal photograph after the picture was taken.

If you save the image to your hard-drive and use a graphic editor to enlarge the view, you can see portions of the fractal image on top of the straps.  Plus there are pixels distortions in several places:
  •  the chain of the necklace
  •  lower right elbow
  •  etc...
